Santé et Psychiatrie is committed to protecting the confidentiality of your personal and health information. This policy explains what information is collected, why, how it is protected, and what your rights are.
Privacy officer
Michelle Carpentier Solorio, NP, acts as the person responsible for the protection of personal information at Santé et Psychiatrie.

Information collected
Identifying information: name, date of birth, contact details, health insurance number where applicable.
Health information: medical and psychiatric history, medication, symptoms, clinical notes and treatment plans.
Payment information: processed directly by our payment processor. No card numbers are retained by the clinic.
Technical information: the site uses a privacy-respecting analytics tool (Cloudflare Web Analytics). It sets no cookies, collects no personal information, and cannot identify you. No advertising trackers are used, and no data is sent to marketing platforms.
The contact form sends your name, email and message through Cloudflare and Resend to the practitioner’s iCloud inbox, solely so that we can reply to you. Cloudflare Turnstile processes technical signals to protect the form against automated submissions. Do not include clinical or urgent information.
Purposes
Your information is used solely to provide care: assessment, diagnosis, treatment, follow-up, clinical communication and billing.
It is never sold, rented, or used for marketing.
Consent
By booking an appointment, you consent to the collection and use of your information for the purposes described above.
You may withdraw your consent at any time, subject to legal obligations to retain the clinical record.
Retention and security
Administrative messages are processed by Cloudflare, Resend and Apple iCloud. They may be processed or stored outside Canada. The form uses HTTPS; messages are not saved in a website database.
The clinical record is retained in accordance with the requirements of the Ordre des infirmières et infirmiers du Québec — a minimum of five years after the last service provided.
Access is limited to the practitioner and, where applicable, administrative staff bound by confidentiality obligations.
Disclosure to third parties
Your information is disclosed to a third party only with your explicit consent, or where required by law — notably in the case of a serious and imminent risk to your life or the life of another.
Our service providers (clinical records platform, payment processor) are bound by confidentiality agreements and host data in Canada.
Your rights
You have the right to access your information, request its correction, withdraw your consent, and file a complaint with the Commission d'accès à l'information du Québec.
Access requests are processed within 30 days.
Privacy incidents
In the event of an incident presenting a risk of serious harm, affected individuals and the Commission d'accès à l'information will be notified without delay, in accordance with Law 25.
